Dear Parent or Guardian,
Santa Clara Unified School District will be participating in the California Healthy Kids Survey (CHKS) administration this year. This is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. The CHKS is a research-based survey that will provide our district and schools with valid indicators to promote student engagement and achievement, safety, positive development, and overall well-being.
Survey content: The survey gathers information on developmental supports provided to youth; school connectedness and barriers to learning, school safety; health-related concerns such as physical activity and nutritional habits; alcohol, tobacco, and other drug use; risk of depression and suicide; and protected class identifiers such as sexual orientation and gender identity.
Student participation is voluntary and anonymous. The survey administration across the district is scheduled for November 6 - 17, 2023. Your student does not have to take the survey. No names or other identifying information except the school's name is connected to the answers. If you do not want your student to participate in the survey, please return the signed bottom portion of the form to your school office by November 14, 2023
